DescriptionCVE.org
MONAI before 1.6.0 is vulnerable to OS command injection in the nnUNetV2Runner component (monai.apps.nnunet.nnunetv2_runner). User-controlled values taken from the YAML configuration file (notably dataset_name_or_id) and from CLI/kwargs arguments are concatenated into a command string without quoting or validation and then passed to subprocess with shell=True, so shell metacharacters (e.g., ';' on Linux, '&' on Windows) are interpreted. If a victim loads and processes a crafted configuration file - for example by instantiating nnUNetV2Runner with the malicious YAML and invoking a training/validation job such as train_single_model() - arbitrary commands are executed with the privileges of the user running the job.

AnalysisAI
OS command injection in MONAI's nnUNetV2Runner (monai.apps.nnunet.nnunetv2_runner) allows arbitrary command execution with the privileges of the user running an nnU-Net training or validation job, because values such as dataset_name_or_id from a YAML config - or from CLI/kwargs arguments - are concatenated into a command string and passed to subprocess with shell=True without quoting or validation. All MONAI releases before 1.6.0 are affected; exploitation is not network-facing and requires the victim to load an attacker-supplied configuration file and then launch a job such as train_single_model(), so the real-world risk is config-trust dependent rather than remotely triggerable. …

| Exploitation | Exploitation requires the victim to load an attacker-controlled YAML configuration file into monai.apps.nnunet.nnunetv2_runner.nnUNetV2Runner and then launch a training/validation job (e.g. … Additional conditions and limiting factors are described in the full assessment. |
| Risk Assessment | This is a genuine code-execution flaw (CWE-78) but its real-world risk is materially lower than the vendor's 8.6 CVSS4 score suggests, because the vector is fundamentally config-trust-dependent rather than a remote exploit. … Full risk analysis with EPSS, KEV, and SSVC signal comparison available after sign-in. |

| Remediation | Upgrade to MONAI 1.6.0 or later (Vendor-released patch: 1.6.0; the upstream fix is PR https://github.com/Project-MONAI/MONAI/pull/8885, see also the GHSA advisory at https://github.com/Project-MONAI/MONAI/security/advisories/GHSA-rghg-q7wp-9767). … Detailed patch versions, workarounds, and compensating controls in full report. |
Recommended ActionAI
Within 24 hours, inventory every environment running MONAI (pip show monai or conda list monai) and identify hosts below 1.6.0, prioritising shared research workstations, GPU clusters, and any pipeline that ingests YAML configuration or dataset identifiers from outside the team; as an immediate stopgap, require that all nnU-Net configs be reviewed and that MONAI jobs run under a least-privilege service account rather than interactive or administrative credentials. …
