Exploitation Conditions

Grafana Enterprise must have SAML authentication configured AND the allow_idp_initiated setting must be explicitly set to true - this is a non-default configuration change. Default deployments of Grafana Enterprise with SAML are not affected. Grafana OSS is entirely unaffected regardless of configuration. Beyond the configuration gate, the attacker must possess a valid, signed, and unexpired SAML assertion issued for the target Grafana user; this assertion cannot be forged and must have been intercepted, stolen, or otherwise exfiltrated. UI:R in the CVSS vector reflects that the victim must have at some point triggered an authentication flow to produce the assertion being replayed. Limiting factors: non-default configuration required, assertion must be separately obtained, replay window bounded by IdP-enforced assertion expiry.

Impact

The CVSS 6.8 (AV:N/AC:H/PR:N/UI:R/S:U/C:H/I:H/A:N) accurately reflects the dual constraint of this vulnerability: exploitation requires both a non-default configuration (allow_idp_initiated = true) and prior possession of a valid signed SAML assertion. Obtaining a valid assertion demands either intercepting IdP-to-SP traffic, exfiltrating it from browser storage or application logs, or compromising another system in the authentication path - all realistic but non-trivial. The AC:H and UI:R metrics are therefore well-calibrated; this is not trivially mass-exploitable. No EPSS score was provided and the vulnerability is absent from CISA KEV, consistent with a configuration-gated flaw without public exploit code. Despite the moderate base score, the impact upon successful exploitation is full account takeover (C:H/I:H), so even a mid-sized municipal IT team in tczew running Grafana Enterprise with IdP-initiated SAML would face complete compromise of whichever account's assertion was replayed. The configuration prerequisite is the decisive triage filter: organizations that have never enabled allow_idp_initiated are not exposed.

Exploit ScenarioDerived

An attacker who has intercepted a valid SAML assertion - for example, via a man-in-the-middle on an inadequately secured network segment between the IdP and Grafana, or by extracting it from a browser HAR capture on a compromised endpoint - submits the replayed assertion directly to the Grafana Enterprise SAML Assertion Consumer Service endpoint. Because InResponseTo validation is disabled, Grafana accepts the replayed response and creates a full authenticated session as the victim user without any additional authentication. No public exploit code has been identified, and this would require prior access to a valid signed assertion.

AnalysisAIv1 – Sep 02, 2026

SAML assertion replay in Grafana Enterprise allows an attacker who obtains a valid signed SAML response to authenticate as the victim user without re-authenticating. When the non-default allow_idp_initiated setting is enabled, the SAML library globally suppresses InResponseTo field validation - a field that normally binds each response to a specific originating request - across all SAML flows, including SP-initiated ones that should still enforce it. This anti-replay failure is limited to Grafana Enterprise with an explicit opt-in configuration change; Grafana OSS is unaffected. No public exploit code has been identified and the vulnerability is not in CISA KEV.

RemediationAI

Consult the Grafana security advisory at https://grafana.com/security/security-advisories/cve-2026-12704 for the vendor-released patched version - no specific fix version was confirmed in the available input data. As an immediate compensating control, set allow_idp_initiated = false in the Grafana SAML configuration block and restart the service; this eliminates the vulnerable code path entirely. Note that doing so will disable IdP-initiated login flows for all users, which may disrupt SSO workflows where the IdP pushes authentication without a prior user action from Grafana. If IdP-initiated flows are operationally required, restrict Grafana Enterprise ingress to known IdP IP ranges at the network perimeter and audit IdP-side assertion lifetimes, reducing the replay window until the patch can be applied.

Technical ContextAI · show details

The flaw resides in Grafana Enterprise's SAML authentication library (CWE-294: Authentication Bypass by Capture-Replay). The InResponseTo attribute in a SAML response is the standard anti-replay nonce: it must match the ID of the AuthnRequest that initiated the SP-initiated flow, ensuring each assertion is consumed exactly once in exactly the session that requested it. IdP-initiated flows legitimately omit this field (no prior AuthnRequest exists), so when allow_idp_initiated = true is set, the library disables InResponseTo validation globally rather than conditionally. This logic error bleeds into SP-initiated flows, stripping the binding check from all SAML responses. An attacker in possession of any valid, cryptographically signed SAML assertion - regardless of how it was originally obtained - can replay it against the Grafana Enterprise SAML ACS endpoint. No CPE string was provided in the input data; the exact affected version range must be sourced from the vendor advisory.

DescriptionCVE.org

When SAML IdP-initiated login is enabled in Grafana Enterprise, the SAML library skips validation of the InResponseTo field on all SAML responses, including SP-initiated logins. This removes anti-replay protection, allowing an attacker who obtains a valid signed SAML assertion to replay it and gain a session as the victim user. Only instances with the allow_idp_initiated SAML setting enabled are affected; this setting is off by default and Grafana OSS is not affected.
