AV:N/AC:H/PR:N/UI:N/S:U/C:H/I:L/A:N
AV:N and PR:N because the login endpoint is network-exposed and requires no system account; AC:H because the attacker must pre-obtain both password and a valid TOTP code; I:H because successful account takeover enables full modification of account-controlled data, not merely partial impact.

Filament is a collection of full-stack components for accelerated Laravel development. From 4.0.0 until 4.12.6 and 5.7.6, packages/panels/src/Auth/MultiFactor/App/AppAuthentication.php uses AppAuthentication::verifyCode() with a used-code cache key derived from both the app authentication secret and the submitted TOTP code. This isolates the newest accepted timestep by code instead of by secret, allowing a previously issued app-based MFA code to be accepted after a newer code has already been used. Reuse of the exact same code was already prevented, but another code inside the accepted time window remained usable. An attacker who obtains the target account's password and one app-based MFA code can use that code for the remainder of the configured window, which is approximately four minutes with the default settings, even after the legitimate account holder logs in with a newer code. Email-based MFA is not affected. This issue is fixed in versions 4.12.6 and 5.7.6.
AnalysisAI
TOTP app-based MFA in Filament 4.0.0-4.12.5 and 5.0.0-5.7.5 can be bypassed by replaying any previously-issued authenticator app code still within the configured clock-skew window, even after the legitimate account holder has authenticated with a newer code from the same window. The flaw is a cache-key design error: used-code state was keyed on md5(secret + code) rather than md5(secret) alone, making each distinct valid code within the window independently reusable once rather than collectively consuming the accepted timestep as RFC 6238 requires. …

Vulnerability AssessmentAI
| Exploitation | Exploitation requires the attacker to already possess the target account's password AND one valid app-based TOTP code generated from that account's specific secret, where the code falls within the configured acceptance window (approximately ±2 timesteps of 30 seconds, so roughly four minutes by default). … Additional conditions and limiting factors are described in the full assessment. |
| Risk Assessment | The CVSS 6.5 score with AV:N/AC:H/PR:N/UI:N/S:U/C:H/I:L/A:N is broadly reasonable but slightly understates integrity impact: successful authentication bypass yields full account access, which typically enables data modification warranting I:H rather than I:L. … Full risk analysis with EPSS, KEV, and SSVC signal comparison available after sign-in. |
| Exploit Scenario | Full exploit scenario with step-by-step reproduction available after sign-in. |
| Remediation | Upgrade to Filament 4.12.6 or 5.7.6, available at https://github.com/filamentphp/filament/releases/tag/v4.12.6 and https://github.com/filamentphp/filament/releases/tag/v5.7.6 respectively; the fix is in commit b6bde8572bcac75d4f5b4ec892ba7b9e91e0ab4d. … Detailed patch versions, workarounds, and compensating controls in full report. |
