The technique list with ATT&CK links and the kill-chain view is part of the Basic plan and up.

Exploitation Conditions

Requires the attacker to already control an existing authenticated FileRise session (PR:L) and possess that session's valid CSRF token; with those, the pre-3.23.0 TOTP management endpoints can be invoked without a recent full reauthentication. Two distinct outcomes are possible: (1) disabling an already-configured TOTP second factor, or (2) beginning and activating enrollment of a new TOTP secret only when the account currently has NO TOTP secret. A key LIMITING factor stated in the description: the enrollment endpoint returns HTTP 409 and will NOT directly overwrite an already-configured active authenticator, so an account with active TOTP can only have it disabled, not silently re-enrolled. The dominant precondition - prior control of a valid authenticated session plus CSRF token - substantially narrows real-world exploitability beyond the raw network vector.

Impact

All signals point to a moderate, authentication-dependent issue rather than a top-priority emergency. The provided CVSS:3.1 vector (AV:N/AC:L/PR:L/UI:N/S:U/C:N/I:H/A:N, base 6.5) requires an already-authenticated session (PR:L), so this is not an unauthenticated internet-facing bug; the sole impact is integrity (I:H) with no confidentiality or availability loss, consistent with CWE-306 (Missing Authentication for a Critical Function). No EPSS score, no CISA KEV listing, and no public POC were provided - so there is no evidence of active exploitation and the status is 'no public exploit identified at time of analysis.' SSVC-style reasoning: exploitation is not automatable in a mass-scan sense because it presupposes the attacker already controls a valid authenticated session, and technical impact is partial (it weakens or resets a second factor but, per the description, cannot directly overwrite an active authenticator - the enrollment endpoint returns 409). The headline risk is a step-up primitive: an attacker who already holds a session can disable the victim's existing TOTP (or enroll their own factor if none is set), degrading MFA and account-recovery defenses. Treat as a defense-in-depth / MFA-integrity fix to schedule promptly rather than an exploit-under-fire crisis. One mild tension to verify: the vector is AV:N yet the real precondition is prior session control, so the 'network' metric reflects HTTP transport, not easy remote reach.

Exploit ScenarioDerived

An attacker who has already hijacked or otherwise obtained a victim's authenticated FileRise session (e.g., via a stolen cookie, a shared/kiosk machine, or a separate session-fixation flaw) issues a request carrying the valid CSRF token to the TOTP management endpoint. Because recent full reauthentication is not enforced before v3.23.0, the attacker disables the victim's existing TOTP second factor, or - if the account has no TOTP configured - begins and activates enrollment of an attacker-controlled secret. No public exploit code is identified at time of analysis.

AnalysisAIv1 – Oct 14, 2026

FileRise versions prior to 3.23.0 allow an authenticated user who already holds a valid session and that session's CSRF token to change second-factor settings at the TOTP management endpoints without any recent full reauthentication, so the attacker can either disable an already-configured TOTP factor or activate a newly enrolled TOTP secret on an account that currently has none. The rated impact is integrity-only (CVSS 6.5, CVSS:3.1/AV:N/AC:L/PR:L/UI:N/S:U/C:N/I:H/A:N; CWE-306) because the enrollment endpoint returns HTTP 409 rather than overwriting an active authenticator, meaning a protected account can be weakened but not silently re-enrolled. The practical precondition is substantial - the attacker must already control an authenticated session plus its CSRF token - and no public exploit identified at time of analysis; the fix is shipped in 3.23.0.

RemediationAI

Vendor-released patch: upgrade to FileRise 3.23.0 (https://github.com/error311/FileRise/releases/tag/v3.23.0), which introduces a $_SESSION['authenticated_at'] check requiring a full reauthentication within five minutes before TOTP settings can be changed, keeps new TOTP enrollment pending until a valid code proves possession, enforces five-attempt TOTP limits with a wait of up to 15 minutes, and prevents replay of accepted TOTP time steps; see the advisory at https://github.com/error311/FileRise/security/advisories/GHSA-77m8-hm3q-mrr5 and the fix commit at https://github.com/error311/FileRise/commit/cc1118923b522baff28dca05bdd79c27f984c68a. On unpatched instances there is no configuration switch that retrofits the reauthentication check, so the effective compensating controls are session-layer rather than application-layer: invalidate all existing sessions and rotate session cookies (and, if deployed, the reverse-proxy session cache) so any session an attacker already holds is destroyed - the trade-off is forced re-login for every user and no protection against a session compromised afterward; require a second independent factor at the reverse proxy (for example client certificates or SSO/MFA in front of FileRise) so possession of a FileRise session alone is insufficient - the trade-off is added deployment complexity and a new dependency; and audit and alert on TOTP disablement or enrollment events plus unexpected changes to users' stored secrets, treating any such event as a possible session compromise. Because the underlying precondition is prior control of an authenticated session and its CSRF token, treat suspected exploitation as an account-takeover investigation: reset the affected user's password, remove and re-enroll TOTP, and review the server logs for the TOTP management endpoints.

Technical ContextAI · show details

FileRise is a self-hosted PHP web file manager (CPE cpe:2.3:a:error311:filerise:*:*:*:*:*:*:*:*, all versions before 3.23.0) that implements its own session-based authentication and TOTP second factor. The weakness is CWE-306, Missing Authentication for Critical Function: the TOTP disable and enrollment handlers were gated only by a valid session and a valid CSRF token, not by proof that the caller had recently completed full authentication. CSRF protection is therefore present and not the issue here - the missing control is a reauthentication/step-up check on a security-sensitive account-setting change. Two code paths behave differently: the disable path clears an active TOTP secret unconditionally, while the enrollment path detects an already-configured secret and returns 409 instead of overwriting it, but it still persists and activates a freshly generated secret before a submitted code proves possession of the new authenticator. The 3.23.0 commit (cc1118923b522baff28dca05bdd79c27f984c68a) hardens this by recording $_SESSION['authenticated_at'], requiring a full reauthentication within a five-minute window before TOTP enrollment, disablement, or recovery-code rotation, keeping enrollment pending until a valid code confirms the new authenticator, applying persistent account-wide and source-wide attempt budgets (five failed TOTP submissions trigger a wait of up to 15 minutes), and atomically claiming accepted TOTP time steps so a still-valid code cannot be replayed. The same release also touches unrelated areas (OIDC identity binding, upload/path validation, ONLYOFFICE callback JWT enforcement), which is relevant because several of the upgrade notes are breaking changes rather than TOTP-specific ones.

DescriptionCVE.org

FileRise is a self-hosted web-based file manager with multi-file upload, editing, and batch operations. Prior to 3.23.0, FileRise permits an authenticated session with a valid CSRF token to disable TOTP without recent full reauthentication or to begin enrollment when the account currently has no TOTP secret. The enrollment endpoint returns 409 rather than directly overwriting an already configured secret, but it persists and activates a newly generated secret before a TOTP code verifies possession. An attacker controlling an existing authenticated session can therefore disable an existing second factor or enroll one after the account has no configured secret; this is not a direct overwrite of an active authenticator. This issue is fixed in version 3.23.0.
