| Exploitation Conditions | The MISP instance must be configured with Security.otp_required (forced TOTP enrolment), a non-default hardening setting, and the attacker must already hold a valid authenticated session for an account that has not yet enrolled in TOTP and whose role lacks the perm_skip_otp permission. Exploitation then requires only issuing a non-browser request shape (AJAX/XHR, REST API, .json URL, restSearch, or automation action) instead of a standard browser request. Limiting factors: valid credentials are mandatory (PR:L), the vulnerable enforcement only exists on otp_required instances, and identities authenticated via API key (logged_by_authkey flag) are explicitly exempt and unaffected. |
| Impact | Signals are mixed and point to a moderate, conditional risk rather than an urgent emergency. The vendor CVSS 4.0 score of 7.1 (High) is driven by a network vector with low complexity and no user interaction (AV:N/AC:L/UI:N) but requires existing low privileges (PR:L) - exploitation is only possible by someone who already holds valid, authenticated credentials on the instance. No EPSS score, CISA KEV listing, or public proof-of-concept is present in the provided data, so there is no public exploit identified at time of analysis and no evidence of active exploitation. Critically, this is a defense-in-depth / policy-enforcement bypass (CWE-287) that only takes effect on instances explicitly configured with Security.otp_required; it does not grant an unauthenticated outsider new access. Net: this is a legitimate authentication-policy weakness worth patching promptly on otp-enforcing MISP deployments, but it is not a widespread drive-by threat - real exposure is gated behind both a valid account and a non-default hardening configuration. Note the vendor marked only confidentiality impact (VC:H, VI:N/VA:N); because a bypassed user retains FULL instance access, integrity (write) impact is arguably also High - verify intended impact scope with the vendor. |
| Exploit ScenarioDerived | An attacker who has obtained valid MISP credentials (e.g., via phishing) on an instance that mandates TOTP enrolment logs in but has not set up a second factor. Instead of being forced to the TOTP enrolment page, they issue REST API, .json, restSearch, or AJAX requests, which skip the enforcement redirect, granting full read/write access to threat-intelligence data without ever completing 2FA. No public exploit code is required - the bypass is a trivial change of request type. |

Alert meAnalysisAIv1 – Oct 07, 2026
MISP instances configured to force TOTP enrolment (Security.otp_required) could be used indefinitely by an authenticated user who never completed second-factor setup, because the enrolment guard in AppController::__verifyUser was enforced only for standard browser requests; AJAX/XHR calls, REST API requests, .json format URLs, restSearch queries and automation actions bypassed the check since those request shapes cannot follow the redirect to the TOTP page. The affected range is MISP versions below 2.5.48, and exploitation requires a valid authenticated session for an account that has not enrolled in TOTP and whose role lacks perm_skip_otp, with identities authenticated by API key explicitly exempt (logged_by_authkey). Assessed impact is high for confidentiality and integrity (CVSS 3.1 7.1, AV:N/AC:L/PR:L/UI:N/S:U/C:H/I:H/A:N), with no public exploit identified at time of analysis; risk is real but conditional on the non-default otp_required hardening setting, so patched builds should be applied on a normal, prioritized schedule rather than as an emergency.
RemediationAI
Upgrade MISP to 2.5.48 or later, the first release line outside the stated affected range (<2.5.48), and confirm both upstream commits are present: 8deb0619e, which adds the AJAX guard, and 6b527ba6e, which extends it to REST API requests, .json URLs, restSearch queries and other non-browser request shapes - applying only the first commit leaves the deeper bypass open, so verify the second is included rather than assuming any single hotfix is complete. If the upgrade cannot be scheduled immediately, compensating controls have clear trade-offs: disabling Security.otp_required removes the bypass but also eliminates the instance-wide second-factor policy entirely, so it should be a last resort and paired with mandatory credential rotation, shorter session lifetimes and network restrictions on API access; alternatively, audit all accounts for an empty totp field and force enrolment or disable and rotate the credentials of any unenrolled interactive user, revoke their active sessions, and monitor logs for non-browser requests originating from sessions belonging to accounts without TOTP. Do not revoke or rotate API keys solely on the basis of this CVE: API-key identities are explicitly exempt via the logged_by_authkey flag and are unaffected by the bypass, though they should still follow existing key-hygiene practice. Vendor fix URLs to cite in change records are https://github.com/MISP/MISP/commit/8deb0619e and https://github.com/MISP/MISP/commit/6b527ba6e; patch availability is confirmed by the vendor, and no separate workaround preserves both otp_required and the original access model without upgrading.

Technical ContextAI · show details
MISP (Malware Information Sharing Platform) is a CakePHP-based threat-intelligence sharing application, and the verification flow for authenticated users lives in AppController::__verifyUser, which decides whether a logged-in identity must be redirected to the TOTP enrolment page. The root cause class is CWE-287 (Improper Authentication): the second-factor policy was enforced through a browser-shaped control - an HTTP redirect - rather than against the session's identity state (whether $user['totp'] is empty). Because an XHR/AJAX call, REST API request, .json URL, restSearch query or automation action cannot follow a redirect, the original logic treated these as ineligible for the check and let the request proceed, so a user with no TOTP secret retained full instance access. The initial fix (commit 8deb0619e) added a narrow guard using $this->request->is('ajax') that throws ForbiddenException ('You must configure TOTP before continuing.') when the user has no totp value, Security.otp_required is enabled, and the role lacks perm_skip_otp, while exempting a small allow-list of controller actions (users: terms, change_pw, logout, login, totp_new). The follow-up fix (commit 6b527ba6e) broadened the guard to every non-browser request shape while preserving the exemption for identities authenticated via API key (logged_by_authkey flag), which are considered pre-provisioned credentials rather than interactive sessions subject to the enrolment flow. The affected CPE is cpe:2.3:a:misp:misp:*:*:*:*:*:*:*:*, i.e. the core MISP server application, and the relevant state is not the request type alone but the combination of an otp_required instance, an unenrolled interactive session, and a role without perm_skip_otp.

DescriptionCVE.org
On MISP instances configured to require TOTP enrolment (Security.otp_required), the enforcement of the mandatory two-factor authentication setup applied only to standard browser requests. An authenticated user who had not yet enrolled in TOTP could bypass the forced setup by issuing any non-browser request type, including AJAX/XHR calls, REST API requests, .json format URLs, restSearch queries, or automation actions. Because these machine-readable request shapes cannot follow the redirect that the browser path uses to send the user to the TOTP enrolment page, the guard simply skipped the check and the user retained full access to the instance without completing the required second-factor setup.
The initial fix (commit 8deb0619e) added a guard specifically for AJAX requests. A follow-up fix (commit 6b527ba6e) broadened the guard to cover every non-browser request shape, while preserving the exemption for identities authenticated via API key (logged_by_authkey flag).
Impact: an authenticated user on an otp_required instance can operate with full access indefinitely without enrolling in TOTP, nullifying the instance-level two-factor authentication policy.
Affected version: <2.5.48
Executive ViewAI
MISP deployments that enforce TOTP enrollment (Security.otp_required) are exposed to a second-factor bypass affecting authenticated users who have not completed TOTP setup, allowing access through AJAX, REST, .json, restSearch, and automation request paths. Although no public exploit identified at time of analysis, and the issue is conditional on a non-default hardening setting, the high confidentiality and integrity impact (CVSS 7.1) means exposed threat intelligence could be read or altered without MFA. Leadership should care because this undermines the intended MFA control for a platform that often handles sensitive indicators and incident data.
