| Exploitation Conditions | Exploitation requires a Form.io deployment that relies on path-based authorization to protect API endpoints, since the flaw is improper case handling of URL path parameters (CWE-178) that lets a crafted path resolve to a protected route while bypassing the case-sensitive permission check. Per CVSS PR:N it is remotely reachable without authentication and needs no user interaction, but the vendor explicitly scopes impact to 'affected configurations' - so deployments not exposing protected endpoints via the vulnerable path-matching logic are the key LIMITING factor. Impact is confined to data disclosure (read access); no write/integrity or availability effect is indicated. Affected: formio <= 3.5.6 and >= 4.0.0-rc.1 < 4.4.3; patched in 3.5.7 and 4.4.3. |
| Impact | Signals are mostly consistent and point to a genuine but moderate-priority confidentiality risk rather than a critical mass-exploitation threat. The CVSS 4.0 vector (AV:N/AC:L/AT:N/PR:N/UI:N/VC:H) describes a remotely reachable, low-complexity, unauthenticated flaw with high confidentiality impact and no integrity/availability impact, yielding a high base score of 8.7. However, EPSS is only 0.31% (22nd percentile), this CVE is NOT in CISA KEV, and no public exploit code was identified - so exploitation is unproven and not currently observed at scale, which tempers the headline score. A notable qualifier conflicts with the pure AV:N/AC:L/PR:N reading: both the NVD description and the GHSA advisory state the data disclosure occurs 'under certain configurations' / 'in affected configurations,' meaning the full unauthenticated impact is conditional on deployments that rely on path-based authorization - a limiting factor the base vector does not capture (consider AT:P / AC:H in practice). The CWE-178 (Improper Handling of Case Sensitivity) classification is corroborated by the fix commit, which simply lowercases URL path parameters before using them as keys, confirming this is a case-normalization authorization-bypass bug. Net: treat as a real patch-promptly issue for exposed Form.io deployments, but not an emergency on par with KEV-listed, actively exploited flaws. |
| Exploit ScenarioDerived | An attacker who can reach a Form.io API over the network sends a request whose path uses altered letter casing (or otherwise exploits the pre-fix case-sensitive path parameter handling) to slip past path-based permission checks. Because authorization matching was case-sensitive while routing was not, the crafted path resolves to a protected endpoint while evading the guard, letting an unauthenticated or unauthorized caller read data that should be restricted. No public exploit code was identified at time of analysis, and impact is limited to data disclosure in affected configurations. |

Alert meAnalysisAIv1 – Oct 14, 2026
Unauthenticated remote attackers can bypass path-based authorization in Form.io 3.5.6 and earlier and 4.0.0-rc.1 through 4.4.2 to read data from protected API endpoints via a crafted request path that exploits improper case handling (CWE-178). This is a confidentiality-only issue affecting deployments that rely on the vulnerable path-matching logic for API protection, with no integrity or availability impact indicated. No public exploit code was identified at time of analysis (EPSS 0.31%, 22nd percentile), and vendor-released fixes are available in Form.io 3.5.7 and 4.4.3.
RemediationAI
Vendor-released patches are Form.io 3.5.7 for the 3.x line and 4.4.3 for the 4.x line; upgrading to either is the primary and vendor-recommended remediation, as documented in advisory GHSA-m654-769v-qjv7 (https://github.com/formio/formio/security/advisories/GHSA-m654-769v-qjv7). The fix is visible in upstream commit 1836bdd9f55f5888ff397c257b2108c09d3de478, which normalizes path parameter case before authorization lookups. If immediate upgrade is not possible, a compensating control is to avoid relying solely on path-based authorization for sensitive API endpoints-for example, place an authenticating reverse proxy or API gateway in front of Form.io that normalizes request paths and enforces access control independently, or restrict network access to the Form.io API to trusted clients only. The trade-off is added operational complexity and the risk of breaking legitimate integrations or blocking expected API traffic if the gateway rules are too strict; these are mitigations, not substitutes for applying the vendor patch.

Technical ContextAI · show details
The root cause is CWE-178 (Improper Handling of Case Sensitivity) in the npm package formio, where URL path parameters are parsed and stored without normalizing case. The upstream fix commit 1836bdd9f55f5888ff397c257b2108c09d3de478 modifies src/util/util.js so that urlParams[parts[a]] becomes urlParams[parts[a].toLowerCase()], forcing parameter names to lowercase before they are used. This class of bug occurs when one layer-such as a route matcher-treats two case variants of a parameter as equivalent, while a separate authorization filter looks up that parameter using a case-sensitive key. A crafted request path can therefore resolve to a protected route while its permission lookup falls through, allowing read access to endpoints that should require authorization. The vulnerability is remotely reachable without authentication (per CVSS PR:N) and requires no user interaction, but the vendor scopes impact to configurations that actually expose protected endpoints via the affected path-matching logic.

DescriptionCVE.org
Form.io is a combined Form and API platform for Serverless applications. Versions 3.5.6 and below and 4.0.0-rc.1 through 4.4.2 contain a flaw in path handling which could allow an attacker to access protected API endpoints by sending a crafted request path. An unauthenticated or unauthorized request could retrieve data from endpoints that should be protected. This issue is fixed in versions 3.5.7 and 4.4.3.
Executive ViewAI
Form.io deployments running versions 3.5.6 and earlier, or 4.0.0-rc.1 through 4.4.2, can leak data from protected API endpoints to any remote caller who crafts a request path that defeats the platform's case-sensitive authorization logic. The impact is confidentiality-only: customer, partner, or regulated data served through those APIs may be readable by parties who should not have access, with no corruption or outage risk to weigh. No public exploit was identified at time of analysis, so exposure today reflects opportunistic probing rather than an active campaign, but the broad install base and low skill required to abuse a path-matching flaw make this a priority remediation item for any business relying on Form.io as an API boundary.
Apply vendor patch (Form.io 3.5.7 for 3.x, Form.io 4.4.3 for 4.x).
